• Vaccines Record
• Social Security Card

Late Arrivals
If you do not cancel or reschedule an appoinment ahead of time, you will be subject to a $30.00 cancellation fee.

Miscellaneous
If you are not the parent or guardian of the patient you must have written consent to have the child seen by us.

Vaccination and exam forms will be done within 48 hours of your request.
